Instructions: Completing the New Library Account Request Form

The Liaison/Assistant Liaison for each district should ensure the following:
  1. Complete the LNOCA Account Request Form.

    At the top of the form:
    - Submitted by
    - District
    - Employee's Name
    - Date
    - (School/Library)Phone
    - Preferred Email Address

    Locate the second large box on the form and check INFOhio.
    - Indicate the primary building for the new library employee
    - If the new employee works in more than one building, list all.
    - Indicate the type of access to be assigned to the account (LIB1, LIB2)

  2. Have the Employee sign and date the form.

  3. Please have the Superintendent (or his/her designee) sign and date the form.

  4. The Liaison and/or Assistant Liaison should then mail/fax the completed form to:
